As a vital member of our multi-disciplinary team, you will coordinate and oversee community-based ageing and disability programs, helping individuals remain in their homes with the support they need. In this role, you will collaborate with a range of health professionals, government and non-government agencies, ensuring clients receive the best possible care.

Key Responsibilities :

  • Coordinate and implement care plans for clients, linking them to medical and community services.
  • Conduct holistic, systematic assessments that prioritise client rights and responsibilities.
  • Manage client care packages, ensuring they are accessible and culturally sensitive.
  • Liaise with external agencies to advocate for clients and ensure their needs are met.
  • Maintain comprehensive case files and care plans, ensuring all documentation is accurate and up to date.
  • Support clients in raising issues and participate in system improvements and service delivery reviews.
  • Manage and report on statistical data to monitor service delivery.

  • Demonstrated experience in case management and working in a multi-disciplinary team.
  • Strong organisational skills and the ability to manage competing priorities.
  • Proven communication and interpersonal skills, with the ability to work with people from diverse backgrounds.
  • Empathy and non-judgmental approach to working with clients, ensuring they receive the best care.
  • Experience working with individuals with complex health needs is highly regarded.
