Description:

Chronic Disease Unit are looking for a Nurse Navigator Nurse Practitioner to join their team. Chronic Disease is a community based multidisciplinary, multi-sited team which offers education, information and support to adults with complex care and / or unstable or complicated diabetes, respiratory disease, chronic heart failure or following diagnosis of a cardiac condition with a strong focus on hospital avoidance and prevention of hospital re-admissions. The service provides care in the home or in a community setting including, group education programs, home visiting and phone consultation services for eligible patients.

About the Role
  • Function in a collaborative model with the healthcare team to provide assessment, diagnosis, treatment and referral as required.
  • Provide safe, quality, cost effective care and practice within the Nurse Practitioner's scope of clinical practice, that is consistent with contemporary standards and evidence-based practice.
  • Improve the delivery of complex patient care, increase system efficiencies, and reduce costs by streamlining and coordinating care.
  • Lead and review processes that support an efficient patient care journey through knowledge of systems and processes enhancing care integration and reducing fragmentation.
