About the opportunity:
Position: Assistant Coordinator
Vacancy: Permanent Part-time, 22.80 hours per fortnight
Location: Home Care Services, Ingham
What's on offer?
Pro rata remuneration package based on full-time equivalent value of up to $103,636 p.a.
Salary range: $41.60 - $45.81 p.h. (OO5)
4 to 6 weeks annual leave with 14%-17.5% loading
12.75% employer contribution to superannuation

About the role
Assist in providing efficient and effective client focused care coordination of eligible Home Care Service clients, in keeping with the requirements of the Workplace Health and Safety Act, Commonwealth Home Support Program.
Liaise with clients, carers, health professionals and community agencies to ensure an effective, collaborative and multidisciplinary approach to the coordination of client care.
Assist in undertaking a range of administrative duties including payroll processing, data entry, staff rostering and accurate data collection for reporting purposes using computer-based programs.
Role requirements
Applicants are advised this position works in an identified Aged Care Facility as per the Aged Care Act 1997 and therefore requires employees and volunteers to have a current National Police Certificate. Queensland Health will facilitate applicants obtaining the above National Police Certificate.
