APS (Australian Partners in Support) is looking to hire a Care Coordinator. in the NDIS and Child Protection space. This role suits an independent worker with extensive experiences in managing clients with challenging behavior.

This role is permanent part time, with flexible working hours.

Role description:

  • Build and maintain positive relationships with staff, clients and their stakeholders
  • Attend and participate in client case meetings as required.
  • Train staff on internal practices and coordinate client specific training as needed
  • Ability to meet potential clients, and promote the services of APS in a positive manner
  • You will be required to ensure care plans are current are communicated with the support team & ensure familiarity with client changing needs and provide direction and support to the care team as appropriate
  • You will play an active role in recruiting staff suitable to meet the needs of our clients
  • Your experience in responding to incident reports, and ensure your documentation is complete will be valuable in this role
  • You will be the appointed Child protection authorized supervisor
  • Maintain all registers and ensure that they are up to date
  • As a Care Coordinator, you will also be responsible for undertaking initial intake assessments, Care Plan development and care needs.
  • Facilitate Team Meetings on a regular basis, facilitate and arrange required training.
  • Perform monthly reviews of client documentation including goal support plans and safety assessments.
  • Be the first point of contact for staff
  • Keeping client files up to date, case noting and reports.
  • Ensure a consistent standard of quality practice.
  • Complete reporting on a monthly basis
  • Ensure services are delivered in line with company policies and procedures and in accordance with the requirements of the NDIS Quality and Safeguard Commission and DCJ.
