Care coordinator / Office Manager
Position type : Full Time
Pay- $95,000- $105,000 per year
Job Responsibilities
The position of Care Coordinator / Office Manager is answerable to their assigned Operations Manager and / or other Senior Management for the day-to-day management of TCASS services. The Care Coordinator / Office Manager is responsible for monitoring support worker skills to ensure TCASS clients are supported to maintain their physical, emotional and intellectual well-being.
Job Description
- Onboard clients and staff to TCASS policy and procedures
- Train and supervise support staff, give regular feedback to support workers regarding performance and further training needs
- Review and update Client Care Plans, Profiles, Life Style Plans and staff routines and procedures
- Monitoring and reviewing of the TCASS quality assurance systems including maintaining and updating the ABC folder system
- Coordination of care for TCASS clients as directed by the Operations Manager
- Management of clients Care Plans and Life Style Plans in conjunction with Operations Manager
- Coordinate client’s medical needs and day to day needs where required in conjunction with support staff support staff.
- Liaison with Case Managers Disability Coordinators, Guardians, families and other Service Providers.
- Available for the on-call roster as required which may involve out of normal hours / weekend work.
Relevant qualifications :
An appropriate tertiary qualification in Aged and Disability, Social and Community studies Current NT drivers licenceRelevant skills
Ability to work in part of a team, display initiative, integrity and a strong work ethic plus organisational and relationship-building skills. High level of organisation skills, good verbal and written communication, liaison skills and ability to work effectively with people from diverse cultural and socio-economic backgrounds. Demonstrated ability to work independently, participate in teams, and follow approaches which value and facilitate client and community participation as a key to achieving this role. Ability to participate in the on-call roster. Demonstrated ability to supervise client caseloads, to meet deadlines and to analyse complex client issues with maturity and well-developed negotiation and problem-solving skills. The position of Team Leader is answerable to the Operations Coordinator and the management team for the day-to-day management of TCASS.
The Team Leader is responsible for ensuring TCASS clients are supported to maintain their physical, emotional and intellectual well-being. The key outcome of this role is to provide a consistent level of quality service provision to TCASS clients and professional development of staff . The needs of the client are paramount but must follow the Occupation Health and Safety Guidelines.
Job Description
Ensure clients are supported adequately with their activities of daily living, personal care, hygiene and daily routine. Advise Operations Coordinator of necessary changes to Care Plans, Client profiles, routines and procedures Follow work health and safety guidelines at all times whilst maintaining a safe environment for TCASS clients and staff alike Record accurate and precise progress notes and complete all required TCASS forms, charts and general documentation Mentor and lead the team towards a common goal of providing quality care for clients Advise Operations Coordinator accordingly regarding staff performance, training and development needs Uphold TCASS quality assurance systems as guided by the ABC folder system and advise Operations Coordinator of changes as required Work under supervision and guidance as recommended by the operations Coordinator and the management team Advise the Human Resources Manager of staff availability, develop rosters and cover shifts where possible at short notice Perform domestic duties such as cleaning, laundry, kitchen duties and other tasks as directed by the management teamEducation Qualification

Planning of care in liaison with the resident Follow communication Protocols at all times when communicating with parents, guardians, stakeholders and other agencies. Communicate respectfully with Clients, parents, guardians, stakeholders, staff members and other generic agencies. Learn each client’s communication methods and familiarise yourself with the use of communication aids that are being used by them. Complete Progress Notes, forms, and other documentation as required. Maintain the client’s Privacy Dignity and Confidentiality at all times. Attend team meetings and other meetings scheduled by the management team. Assist clients with their medical and other appointments. Safe use and care of all residents’ personal property Showering, grooming and medication Bowel care, Pressure care, catheter care and physio exercises Meal preparation and Feeding Encouragement and support with community access activities.
