You will provide expert general and clinical pharmacy services to renal dialysis patients at Redland Hospital, mainly in dialysis and outpatient settings. The role operates independently, offering clinical interventions, advice, workflow management, and service development.

About the Role
- Deliver high level knowledge in renal clinical pharmacy services by applying independent professional judgement to a complex caseload, adapting routine clinical practice based on valid and reliable evidence.
- Deliver high level specialist clinical pharmacy service in the ambulatory setting for chronic condition management, which may include dispensing, medication reconciliation, drug information, counselling, and recommendation, incorporating safe transition of care across the continuum of care.
- Continually develop clinical practice knowledge, skills and expertise through self-directed learning, active learning within a team environment and engaging in professional development activities, including participating in the performance appraisal and development plans.
- Demonstrated ability to provide Renal Pharmacist clinical services of a complex nature including rapid prioritisation, and management, of patients in a complex clinical environment.
