Role Overview:
As a Disability Support Worker at Hobart City Mission, you will provide person-centered care, empowering individuals with disabilities to lead independent and meaningful lives. You will work closely with clients to assist them with their day-to-day activities, personal care, and community engagement, while fostering a safe and supportive environment. This role is ideal for individuals who are passionate about making a difference and who thrive in a rewarding, client-focused environment.
Key Responsibilities:
Personalized Support: Provide hands-on support to individuals in daily activities such as personal care, meal preparation, and household chores, tailored to meet each person’s unique needs and preferences.
Community Participation: Encourage and assist clients in engaging with their local community, whether through recreational activities, social outings, volunteering, or family interactions, ensuring inclusion and social connection.
Promote Independence: Help clients develop essential life skills, increase their self-confidence, and support them in achieving their personal goals, promoting independence and choice in all aspects of their lives.
Maintain Client Records: Accurately document client progress, needs, and any incidents to ensure continuity of care, maintaining a high standard of record-keeping in compliance with NDIS guidelines.
Collaborative Teamwork: Work closely with other support workers, management, and external health professionals to ensure clients receive holistic and well-coordinated care.
Safety and Well-being: Maintain a safe, respectful, and compassionate environment for all clients, ensuring that any risks are identified and managed appropriately.
Skills & Qualifications:
Certificate III in Individual Support, Disability, Aged Care, or a related field (preferred).
NDIS Worker Screening Check and completion of the NDIS Orientation Module (required).
First Aid and CPR certification (desirable).
Previous experience in disability support or a similar role is an advantage but not essential.
A strong understanding of the NDIS framework and a person-centered approach to care.
Excellent communication skills and the ability to build positive relationships with clients, families, and team members.
Patience, empathy, and a genuine desire to help others improve their quality of life.
Ability to work both independently and collaboratively in a team environment.
