Job Description

About the role

The Brighter Futures Practitioner provides tailored case management services to young adults leaving care to ensure they are supported through their transition, and to help them in their journey to living as independent adults. They are responsible for effective engagement with young adults, their families, and their support network to identify their needs and aspirations. They will also develop and implement a Care Plan created and led by the young adults, incorporating the administration of eligible financial supports.

Duties

  • Deliver services and supports, ensuring activities are aligned with the program model of care.
  • Build client agency, self-efficacy and facilitate change through client empowerment, while actively discouraging client dependence on supports.
  • Maintain accurate and detailed case notes of all client interactions.
  • Lead/participate in internal and external stakeholder and case review meetings with community and government agencies for clients with multiple needs.
  • Engage all allocated client cases, ensuring initial outreach home contact and consent is gained in a timely manner.
  • Develop clear goals and Care Plan, based on the client-led Assessment.​​​​​​​
  • Deliver targeted Motivational Interviewing interventions for each client, as per the early stages of engagement.​​​​​​​
  • Deliver targeted family connection and family mediation sessions between young person and their caregivers/adult kin in building positive family relationships.

  • Multiple Permanent Full-time position.
  • Starting Salary of $86,858 per annum (depending on qualification and experience).

Desired Skills and Experience

COVID-19 Requirements

  • Immunization history showing you have received two doses of a Covid 19 vaccine is encouraged.

Required

  • Current Bachelor’s Degree, in Social Work, Human Services, Social Science, Health, Allied Health, Youth Work, Community Services, Family Intervention or Criminology with 2 years’ relevant experience in social services field.

OR

  • Current Diploma in Community Services, Social Work, Human Services, Child and Family, Allied Health, Youth Work, Child and Family Intervention with 3 years’ relevant experience in social service field.
  • AND at least 1 year of abovementioned experience in a case management role.
  • Demonstrated ability and comfortableness working in a performance driven environment to meet individual weekly, monthly and quarterly performance targets.
  • Demonstrated ability to build client agency, self-efficacy and facilitate change through client empowerment, while actively discouraging client dependence on supports.
  • Demonstrated knowledge and ability to provide behaviour change interventions that build client self-awareness and active change in achieving their identified goals.
  • Demonstrated ability in coordinating multiple services in providing access to holistic care for clients, particularly integrating internal agency supports across multiple teams.
  • Demonstrated knowledge and understanding of the factors contributing to vulnerability, disengagement, and intergenerational trauma of client group.
  • Experience with case management for high risk and/or crisis clients, with high-level planning and organisational skills.
  • Knowledge of the supports available to target clients in the areas of mental health, substance misuse, education, employment and housing.

Additional Requirements

  • Requirement to travel across the Logan and/or Brisbane region, as per program location.
  • Current drivers’ licence
  • Flexibility to work outside 8:30am-5pm business hours in meeting individual client needs and provide responsive care.
  • Working with Children’s Card (Blue Card) or be eligible to apply.
  • Valid Police Check.
