Your Opportunity
Support individuals and families in our community at risk of homelessness. Not just during a crisis - but throughout their journey towards independence.
Based in Meekatharra, the Housing Case Manager will support families, women, and their children to achieve their goals in a safe and secure environment, improving their health and wellbeing. You will work under general direction, applying specialised knowledge and expertise to support clients in overcoming barriers related to housing tenure, homelessness, education, or employment opportunities. This role is part of the Homelessness & Housing Support service – Meekatharra, which provides a transition pathway for families, women, and children seeking to move out of unsafe or unstable housing situations with case management and community development support.
Your Key Responsibilities
- Conduct intake assessments, pre-tenancy interviews (PTI), and manage the housing/tenancy waitlist.
- Provide wraparound support services for women and children in need, assisting them in integrating into mainstream housing through advocacy, case management, and referrals to relevant agencies.
- Assist clients in establishing relationships, understanding their immediate and long-term needs, and supporting them in their housing journey.
- Work alongside the Department of Housing, advocating on behalf of clients and ensuring tenancy outcomes are achieved.
- Facilitate and develop relationships with internal and external stakeholders, including attending local network meetings.
- Support clients with tenancy management, including assisting with moving-in processes, setting up essential services, arranging NILS loans for household items, and ensuring properties remain well-maintained.
- Maintain comprehensive case notes, complete reports, and provide accurate client documentation, including referrals, statistics, and service reviews.
With a significant background in community and/or human services sectors, you will be confident in your case management and assessment processes. Other requirements include:
- Relevant degree with at least one year’s experience, Associate Diploma with relevant experience, or less formal qualifications with substantial relevant experience.
- Demonstrated experience working with diverse clients, including Aboriginal and Torres Strait Islander and Culturally and Linguistically Diverse (CALD) communities.
- Strong understanding of housing instability, homelessness, and the challenges people face, including alcohol and other drugs, family and domestic violence, mental health, and social/cultural obligations.
- Experience in trauma-informed case management, with the ability to work autonomously and manage up to four cases simultaneously.
- Excellent verbal and written communication skills, as well as strong advocacy, stakeholder engagement, and relationship-building abilities.
- Ability to work within a multi-disciplinary team and remote setting, exercising judgment and managing multiple tasks under time pressures.
- Proficiency in Microsoft Office and ability to adopt Mission Australia’s web-based Information Management System (training provided).
- Full unrestricted driver's licence.
