The Transition Care Program provides a range of short-term care services to eligible older people following a hospital stay to help them regain as much independence as possible.
Services include therapy, nursing support and /or personal care that will help eligible older people regain as much independence as possible and assist them in making long term care and support arrangements.
About the role:
Reporting to the Transition Services Manager, this role will be involved in coordination of client services and provision of discipline specific therapy, assessment and intervention.
Duties for the role will include:
- Providing written information to staff, patients and family/carers regarding Transition Services,
- Actively collaborate in early and timely multidisciplinary discharge planning,
- Clear and accurate reports, presentations and contribution to publications regarding the program operation, outcomes and its objectives.
About you:
- Demonstrated skills in case management/care coordination and the ability to make complex clinical decisions including sourcing and coordinating services to meet clinical and community care needs,
- Ability to contribute to plans, develop, implement and evaluate community oriented, holistic therapeutic interventions appropriate for older people and their families,
- Strong ability to work with clients who have diverse value systems, cultural differences and special needs
- (Physiotherapist) - Appropriate Degree or equivalent qualification which entitles registration with the Physiotherapy Board of Australia.
- (Occupational Therapist) - A degree or equivalent qualification which entitles registration as an Occupational Therapist with the Occupational Therapy Board of Australia
- (Social Worker) - Appropriate Degree or equivalent qualification which gives eligibility for full membership of the Australian Association of Social Workers.
