We are seeking a dedicated and compassionate Coordinator, Wellness & Reablement Services to lead the admission process and coordinate services for clients under the Commonwealth Home Support Program (CHSP). In this role, you will work closely with clients, carers, and internal teams to ensure clients access the right services, helping them achieve improved health outcomes and enhanced quality of life.
Key Responsibilities:
- Oversee the client onboarding process, ensuring accurate assessments, care planning, and timely service delivery.
- Collaborate with clients and their carers to provide advice, education, and support in accessing appropriate services.
- Continuously assess, plan, implement, and evaluate actions to meet clients' individual health needs and preferences.
- Ensure all service delivery aligns with organisational values, policies, and procedures.
- Adhere to budgetary constraints, accreditation standards, compliance, and relevant legislation while delivering services.
- Contribute to team management efforts to balance client volumes, service outputs, and contractual obligations.
- Monitor service delivery to ensure it meets the high standards of care expected by our clients and partners.
Requirements
Key Skills and Qualifications:
- Previous experience in a health or community services coordination role (preferred).
- Strong understanding of the Commonwealth Home Support Program (CHSP) and related legislation.
- Excellent communication and interpersonal skills, with the ability to collaborate effectively with clients, carers, and team members.
- Ability to work independently and as part of a team.
- Highly organised with strong attention to detail and time management skills.
- Commitment to person-centred care and a passion for supporting individuals to live independently and well.
