Where you'll be working
The Team office is located at Macquarie Hospital North Ryde; however, the care is provided across NSLHD to people in their own homes.
What you'll be doing
Enhancing Supportive Community Care (ESCC) is a collaborative community-based multidisciplinary team which provides care to people with late-stage degenerative and chronic conditions, and disability. The service model is designed for people in the last 2 years of life with a focus on wellness and prevention of hospitalisation.
The ESCC multidisciplinary team will work in partnership with Northern Sydney Home Nursing Service (NSHNS), General Practitioners and other health service providers. The Department Head is a Senior Allied Health Clinician with expertise in supportive care to those in the last 2-3 years of life, providing consultancy service across NSLHD. The role will be accountable for the development and provision of the teams of high quality clinical services in response to demand and client needs.
The Department Head provides leadership, facilitates evidence informed clinical practice and evaluation for the identified consumer cohort, which is innovative, strategic, and sustainable. They will be responsible to lead and manage a small multi-disciplinary team in the delivery of appropriate community care for patients, as referred in a manner consistent with professional code of ethics and current Northern Sydney Local Health District (NSLHD) policies and procedures.
The Department Head will provide leadership, management, and accountability for staff, and will ensure clinical practice is consistent and in accordance with evidenced based practice. The incumbent will liaise closely with the community nursing to ensure seamless integration and coordination of day-to-day services and provide appropriate advice and support as required. The Department Head will work Monday to Friday and carry a clinical caseload.

Candidates will need to meet the following criteria


  • Senior Psychologist or Occupational Therapist with full AHPRA registration or Social Worker eligible for membership of AASW. Social Workers and Occupational Therapists will need minimum experience as a Level 2. Senior Psychologist with a four year degree in psychology, registered as a Psychologist with the Psychology Board of Australia and a minimum of one year at the 9th year of service on the Psychology salary scale.
  • Demonstrated experience effectively leading and managing the operations of a program and/or a multi-disciplinary team.
  • Significant level of clinical expertise relevant to people living with late stage degenerative and chronic conditions and disability, and knowledge and understanding of contemporary models of care and service delivery.
  • Current NSW Drivers Licence.
