MENTAL HEALTH SUPPORT WORKER
Due to rapid growth, we are seeking multiple Mental Health Support Workers to join the Brave Minds Community Access Team.

We are currently seeking complex mental health support workers that have experience working alongside people with psycho-social disability to
- Access essential services
- Build professional relationships with clients to understand personal life goals, independent abilities and opportunities.
- To support and empower clients to achieve the things that matter most to them.
- Aim to increase a persons quality of life.
Essential Requirements
- A comprehensive understanding of mental health issues and the challenges individuals face
- Be passionate about pursuing a people focused career which genuinely impacts the lives of others.
- A thorough understanding of recovery and rehabilitation using a strength based and solutions focused practice
- Understanding of person-centred support
- Demonstrated experience working with individuals who live with a mental illness.
- Tertiary qualifications in a related field, such as Youth Work, Social Work, Human Services, Community Mental Health OR similar, or relevant experience working alongside people with severe, persistent psychosocial disability (minimum of 5 years)
- The ability to plan, implement, deliver and evaluate plans that work towards the individual’s NDIS goals
- Ability to work independently and collaboratively in the community and as a team member
- Great communicators dedicated to providing the best service for every person
- A broad understanding of mental health systems and legislation
- A Disability Positive Notice card (yellow card)
- A Children Check Positive Notice card (blue card)
- A current first aid certificate
- A current CPR certificate
- A current driver’s license

Tasks & responsibilities
- Provide a person-centered approach with each individual.
- Be aware of people’s goals, as identified by their NDIS plan and work with people to determine a course of action to work towards them.
- Use initiative to work towards goals of individuals.
- Assist and support individuals to understand what is needed to obtain optimal well-being.
- Contribute to and implement best practice initiatives based on the need of the individual.
- Work with wider stakeholder team to work towards optimal outcomes.
- Provide case notes for each person at the end of each shift.
- Provide emotional, practical and general support using interpersonal skills to assist individuals to resolve their own problems.
- Provide practical support, advocacy and resources when needed.
- Work under general direction and use own knowledge, judgement and organisational skills.
- Manage and plan your own work.
- Assist clients to access relevant community services and programs as needed.
- Providing timely and accurate case notes.
- Provide support to work towards individual goals.
- Assist and monitor medications.
- Support the development of daily living skills and capacity for self-care.
