PsychPhys is seeking an Advanced or Proficient Behaviour support Practitioner, positioned for growth across our South East catchment. The Role:

Advanced or Proficient Behavior Support Practitioner plays a key role in providing mentorship and Proficient Behavior Support Programs and Services tailored to clients within the South-East catchment area. The role consists of mentorship, 1:1 Behavior Support, multi-disciplinary Psychoeducational support; developing, writing; and implementing Behavior Support Plans. The role works in collaboration with a multidisciplinary team in the delivery of tailored services . The ongoing opportunity is full time and based across our Surrey Hills and Frankston practice sites.

Responsibilities:

  • Conduct initial Program Screening Assessments
  • Provide 1:1 Behavior Support/consultation to clients
  • Support young people with social and/or high-risk behaviors
  • Conduct Risk Assessments and initiate harm minimisation strategies and techniques, including Suicide and Violent risk
  • Provide Mentorship and peer consultation to Behavior Support Practitioners
  • Conduct Functional Behavior Assessments
  • Written reports
  • Work in collaboration with Multi-Disciplinary Team in the facilitation of programs to individuals and their families
  • Initiate/attend Care Team Meetings
  • Develop and implement Behavior Support Plans and report on and monitor regulated restricted practices
  • Deliver multi-disciplinary Psychoeducational group sessions if/when required
  • Provide Psychoeducation to key stakeholders, care teams and family members
  • Comply with all Professional Development and supervision requirements as per operational requirements

Candidate:

  • Demonstrate the ability to deliver and communicate sensitive information with individuals inclusive of culture, age and cognition.
  • Demonstrated ability in developing and implementing Behavior Support Plans
  • Demonstrated experience in Restrictive Practices and code of ethics,
  • Demonstrated compliance with the PBS Frameworks, NDIS Quality and Safeguard Commission.
  • Demonstrate organic and genuine rapport building with individuals and stakeholders.
  • Demonstrated high level organisational skills - time Management, goal setting, delegation, self motivation, attention to detail, analytical thinking and communication
  • Demonstrated ability to offer calm response under pressure
  • Demonstrated ability to identify risk and ensure mandatory reporting practices are initiated (where required)
  • Demonstrated effective verbal and written communication with particular emphasis on interpersonal communication skills and qualified written work
  • Demonstrated experience working within community health
  • Demonstrate experience in the provision of evidence-based assessment and interventions.
  • Demonstrate the ability to maintain confidentiality and record-keeping.
  • Demonstrate skill in working as part of a multidisciplinary team.
  • Demonstrate competence in the use of information technology, internet, and desktop applications; e.g. electronic client record systems, Google Workspace and database packages.
  • Enjoys health and fitness, and promotes benefits of healthy independent living.

  • Demonstrated strong commitment to improving health outcomes in individuals.
  • Understanding of emotional intelligence and the ability to adapt to the individual as required.
  • Ability to work autonomously and effectively self managed time strategies when needed to work with minimal supervision.
  • Enjoys working in a team environment and a collaborative approach to therapy.

Qualifications:

  • A tertiary qualification in Community Services, Nursing, Teaching, Social Work, Speech Pathology, Occupational Therapy, or other relevant health/community services certification/qualification and/or relative working experience.
  • A satisfactory Police Criminal Check dated within 6 months of commencement.
  • A valid Employee Working with Children’s Check.
  • A current Driver’s License
  • Access to a vehicle for travel.
  • A valid NDIS Worker Screening check.
