About the role
Are you a passionate and dedicated nurse eager to make an impact in a rural community? South West Healthcare (SWH) is looking for a Community Health Nurse to join our Macarthur Community Health Centre team, where you will provide outstanding care to local clients.
In this role, you will work alongside a multidisciplinary team to deliver holistic care, conducting detailed assessments, crafting personalised care plans, and evaluating health outcomes. Through effective communication and collaboration, you'll foster meaningful connections that empower clients to take charge of their health and well-being.
Position overview
- Permanent, Part Time (32 hours per fortnight)
- Macarthur Community Health
- Community Health Nurse
- Salary: $107,390 per annum, pro rata, + superannuation
What you bring
- Registered Nurse with current registration (Australian Health Practitioners Regulation Agency, (AHPRA)
- A minimum of 5 years' experience in acute nursing, with demonstrated skills in clinical and holistic assessments
- Strong communication abilities and a commitment to delivering exceptional patient care
- Proactive, compassionate, and collaborative approach to client care
