Support Worker Position Description (but not limited to):

  • Support NDIS participants to engage in activities and tasks including capacity building, in-home support, and community access within the South Eastern and Eastern areas of Victoria
  • Support NDIS participants to access the community.
  • Monitor the safety and well-being of the participant and report any concerns to the Team Lead.
  • Ensure completion of appropriate documentation such as participant progress notes, incident reports, and goal attainment using ShiftCare and the Client Management System.
  • Undertake relevant participant-specific and mandatory training modules to maintain NDIS practice competency requirements.
  • Actively contribute to a team environment through open communication, participation in regular team meetings, planning processes, and professional development /supervision.
  • Provide backup and support to other staff, as necessary.
  • Report any quality and safety incidents or hazards at work immediately to the Team Lead.
  • Carry out their roles and responsibilities as detailed in the relevant policies and processes.

Qualifications – preferred

  • Cert IV Child Protection, Mental Health, Disability, Community Services, or similar or relevant tertiary qualification or experience in the field.
  • Current National Police Check.
  • Working with Children Check.
  • NDIS Workers Orientation (or willing to complete).
  • Current Driver's license and fully comprehensive insurance.

Competencies and experience:

  • Experience working in the disability field.
  • Experience working with people with disability who have been involved in the criminal justice system, and behavior of concerns (preferred).
  • Genuine passion for the field of work.
  • Strong engagement, enthusiasm, resilience, and self-care.
  • Experience in medication administration or willingness to be trained.
  • Understanding of Behaviour Support implementation and restrictive practice (mandatory).
