Support Worker Position Description (but not limited to):
- Support NDIS participants to engage in activities and tasks including capacity building, in-home support, and community access within the South Eastern and Eastern areas of Victoria
- Support NDIS participants to access the community.
- Monitor the safety and well-being of the participant and report any concerns to the Team Lead.
- Ensure completion of appropriate documentation such as participant progress notes, incident reports, and goal attainment using ShiftCare and the Client Management System.
- Undertake relevant participant-specific and mandatory training modules to maintain NDIS practice competency requirements.
- Actively contribute to a team environment through open communication, participation in regular team meetings, planning processes, and professional development /supervision.
- Provide backup and support to other staff, as necessary.
- Report any quality and safety incidents or hazards at work immediately to the Team Lead.
- Carry out their roles and responsibilities as detailed in the relevant policies and processes.
Qualifications – preferred
- Cert IV Child Protection, Mental Health, Disability, Community Services, or similar or relevant tertiary qualification or experience in the field.
- Current National Police Check.
- Working with Children Check.
- NDIS Workers Orientation (or willing to complete).
- Current Driver's license and fully comprehensive insurance.
Competencies and experience:
- Experience working in the disability field.
- Experience working with people with disability who have been involved in the criminal justice system, and behavior of concerns (preferred).
- Genuine passion for the field of work.
- Strong engagement, enthusiasm, resilience, and self-care.
- Experience in medication administration or willingness to be trained.
- Understanding of Behaviour Support implementation and restrictive practice (mandatory).
