Remote Support Coordinator Level 1/ Level 2
We are currently seeking a vibrant and experienced Level 1 or Level 2 Disability Support Coordinator to join our team on a casual basis (1 day per week) with a view to full-time. Reporting to the Business Development Manager, the Support Coordinator will be responsible for:
Locating, managing and connecting a range of supports and stakeholders to meet participant needs.
Ensuring care plans reflect participant independence, as well as supporting social and economic participation.
Monitoring and evaluating services in line with participants evolving requirements and ensuring support are working towards and meeting participant goals.
Working collaboratively with participants, providers, stakeholders and family to ensure holistic support provision.
Attending community and networking events to source and on-board new participants.
Supporting participants to understand their plan and support available to them.
Adhere to NDIS documentation and reporting standards.
The successful candidate will have:
Minimum 2 years’ experience in a similar role with sound understanding of NDIS guidelines and practices - This is essential.
Excellent communication and interpersonal skills.
Confidence and proficiency sourcing and on-boarding participants.
Ability to problem-solve and achieve monthly KPIs.
Unrestricted Australian driver’s license and access to own vehicle.
