About the Role:
This is an exciting opportunity to embark on an enriching career as a Transitional Care Consultant within the Northern Adelaide Local Health Network's Mental Health division. The Northern Adelaide Local Health Network (NALHN) is a leading provider of public healthcare services in South Australia.
As a Transitional Care Consultant, you will play a critical role in ensuring seamless transitions for consumers within the mental health service system. Your expertise will bridge the gap between inpatient and community-based care, fostering a collaborative approach that prioritises consumer well-being and recovery.
In this rewarding position, you will leverage your strong interpersonal skills to build trusting relationships with consumers, their families and healthcare professionals. Your ability to navigate complex systems and advocate for individualised care plans will be instrumental in achieving positive outcomes.
To be eligible for this position as an Allied Health Professional , you must hold an appropriate degree or equivalent qualification for one of the following Allied Health disciplines working in the service, specifically:
- Appropriate Degree or equivalent qualification which entitles registration as an Occupational Therapist with the Occupational Therapy Board of Australia.
- Appropriate Degree or equivalent qualification which gives eligibility for full membership of the Australian Association of Social Workers.
- Appropriate Degree (Masters in Clinical or Health Psychology) or equivalent qualification as recognised by the PsyBA and general registration with PsyBA. Must hold an area of practice endorsement and approved supervisor status with the PsyBA at all levels of supervision (primary and secondary).
To be eligible for this position as a Registered Nurse , you must be registered as a Nurse by the Nursing/Midwifery Board of Australia and who holds a current practising certificate. Must be enrolled in an approved mental health course or hold a qualification in mental health practice.
