We’re looking for a dedicated Direct Care Worker (Community) to support people who are aged, frail, or disabled and wish to remain living independently in their own homes. About the Role:
The Direct Care Worker (Community) plays a key role in providing direct client services to individuals who require assistance with daily activities to remain living in their own homes. You will be working closely with a Community Care Coordinator and collaborating with a multidisciplinary team of Community Health professionals to ensure the best outcomes for our consumers.
In this role, you will deliver a range of personal and practical care services, tailored to the individual needs of each client, while promoting their independence. You’ll assist clients with activities such as personal hygiene, mobility, meal preparation, and other daily living tasks, ensuring they are treated with respect and empathy. You will also provide support to families and carers, helping to navigate the care process and empowering clients to live fulfilling lives in their chosen environment.
Working as part of a multi-disciplinary team, you will be an integral part of providing holistic, high-quality care to those in need, all while adhering to regulatory standards and organizational guidelines.
What We’re Looking For:
- Experience in exercising own judgment and initiative in the day to day execution of a position.
- Experience in the use of computer packages eg. Microsoft Word, Excel.

- DHS National Disability Insurance Scheme (NDIS) Worker Check
- National Police Certificate (NPC) for employment involving unsupervised contact with vulnerable groups required for this position (E.g. aged care employment or work involving vulnerable clients)
- Immunisation Risk for this position is - Category A
