The Department
Health of Older People
The Health of Older People Geriatric Evaluation and Management Units at Caulfield specialise in caring for older people requiring further assessment, rehabilitation and complex discharge planning. We are recruiting to two Clinical Support and Development Positions – which will be aligned with either Ward ACG (15 bed GEM and 15 bed secure Delirium Dementia unit) or AC1 (30 bed GEM unit). The units are very dynamic environments with a diverse complex patient groups. Our passionate and highly skilled multi-disciplinary teams work very closely together to achieve excellence in quality patient care.
The Role
The Clinical Support & Development Nurse works closely with the Nurse Manager and Ward Governance team to promote excellence in practice in subacute care settings and ensure optimal outcomes for patients. The role also focuses on mentoring and coaching for nursing staff, ensuring high standards of patient care, and promoting professional pathways for nurses in subacute clinical environments.
Key Responsibilities
- Provide education, support, and mentorship to support professional development for nursing staff in subacute care settings
- Promote continuous learning and inquiry to improve nursing practice
- Develop programs and strategies to monitor and improve the quality of nursing care, patient outcomes and staff experience
- Foster clinical leadership in the delivery of exceptional subacute nursing practice
Qualifications and Skills Required
- Current registration as a Registered Nurse with the Nursing & Midwifery Board of Australia via AHPRA
- Working towards or holding a Master of Nursing or Education
- Proven experience in education roles and clinical leadership within subacute care
- Extensive clinical knowledge and expertise in subacute nursing
- Strong understanding of nursing scope of practice, supervision, and delegation in subacute settings
