About The Role
Sydney Local Health District has a long and proud history of delivering care in the community including prevention, early intervention, assessment, treatment, health maintenance and continuing care services designed to improve and support the health and wellbeing of individuals and communities.
To provide health care services (community development, health education, group work, visual arts, outreach activities and programs) to marginalised and ‘at risk’ young people with the team of linking young people to services (health, education, employment), through:
Working creatively to engage with the target group, particularly those young people not linked in with services.Accessing and linking with young people through the Intake officer.Consulting with ‘at risk’ young people and relevant community members and services to identify their needs and strategies to address what they are. Developing creative, innovative programs appropriate for ‘at risk’ young people to address their needs.Providing a timely and efficient counselling response to young people who request this.Linking young people to mainstream health and other appropriate services.Providing case management to ‘at risk’ young people within the health service.

Ideal Candidate
Relevant health and/or community qualifications and/or experience and demonstrated knowledge and experience working with young people and families.Demonstrated experience in engaging and working effectively with young people and other marginalised young people and an understanding of issues affecting their health and wellbeing.
