The Salvation Army (Salvos) is one of the largest providers of homelessness services across the country. Our philosophy is that every person should be afforded dignity and respect and that no one should be without a safe, affordable, and secure home. We provide services and supports to adults, and families with accompanying children in the form of accommodation, case management, assertive outreach, financial assistance, community connection and referral services.

About the role

We are seeking a values-driven Case Manager – Homelessness Support Services. The primary purpose of the Case Manager position is to provide case management support outreach to individuals and families with multiple and complex needs who are homeless or at risk of becoming homeless.

Reporting to the Team Leader – Homelessness Support this is a Permanent Part time, 60.8 Hours per fortnight (30.4 hours per week) role, located in Shepparton, VIC.

  • Review referrals to ensure eligibility, participate in allocation, and perform outreach to meet local targets.
  • Client needs assessment: Conduct initial and ongoing assessments to understand the client's issues, needs, and circumstances.
  • Implement tailored case management processes and support clients in setting and achieving personalized goals, including monitoring and reviewing
  • progress.
  • Help clients access appropriate services, establish community supports, and advocate for them when needed.
  • Identify service gaps and provide feedback to the team leader for potential improvements in service delivery.

You will have

  • A Tertiary qualification in Social Work, Welfare, Community Development or a related field
  • Demonstrated knowledge and experience in Case Management
  • A current Drivers Licence
  • A current and valid working with children’s check
  • A national police check (which will be undertaken by the Salvation Army if application proceeds).
  • Experience in Client assessment including appropriate information, tools and techniques to gather and analyse information
  • Relevant experience in using appropriate risk models to assess levels of risk, harm and well-being, including immediate risk
  • Empathy: Strong interpersonal skills with the ability to connect with (xxx) youth from diverse backgrounds.
  • Interpersonal Skills : Strong interpersonal and communication skills, with the ability to build trusting relationships.

Conditions

For Award Based roles:

Compensation is in accordance with SCHADS Award Level 5.

$49.28 - $50.40 hourly + superannuation based on experience level and qualifications, exclusive of penalties, allowances, NFP salary packaging.
