About The Role
We have an exciting opportunity for a full time NDIS Support Coordinator; fixed term 12-month maternity cover, to join our team in Alice Springs. The Support Coordinator will assist people with a National Disability Insurance Scheme (NDIS) plan to make choices of services and supports, coordinate the provision of those services and supports, monitor the delivery of same and provide regular reports to required agencies. Work as part of a local and dispersed team to realise the needs, wants, desires and aspirations of the people we support.
The Support Coordinator will provide this assistance to people based across Somerville housing in the NT.
Core Duties May Include But Are Not Limited To
Work with the participant and (where relevant) their support network including family / guardian to identify supports / programs / activities / services that will enable the participants’ needs, wants, desires and aspirations to be met within the allocated funding of the NDIS plan.Obtain quotes for services and present to the participant for consideration.Completes referrals to relevant services.Strengthen and enhance the participant and their support networks (family / guardian’s) abilities to coordinate supports. Build the capacity of the participant to achieve greater independence to self-direct services and supports in the longer termEnsure mainstream services meet their obligations to the participant (i.e. housing, education, justice, health)
Desirable Criteria
Previous experience or relevant qualification in community services, or a similar roleUnderstanding of National Disability Insurance Scheme (NDIS)Demonstrate ability to plan, prioritise and execute tasks efficiently.Excellent communication and collaboration skills with internal and external stakeholdersA genuine desire to impact positively on peoples lives.
