Why This Role Matters
The Open Door and Flagstaff provide short-term accommodation for men experiencing homelessness and complex needs, with Open Door housing 37 men and Flagstaff 64. Both offer case management and outreach support through a transdisciplinary team to help clients achieve sustainable housing outcomes. The Transitional Housing Case Manager provides care coordination and case planning, working with clients to address housing and support goals while promoting independence and dignity. The role also includes outreach and facilitating “warm” referrals for clients transitioning from crisis accommodation.
About The Role
We are seeking a dedicated Case Manager for our Transitional Housing program in a Full-time capacity. This position is based out of Flagstaff
Salary and conditions are in accordance with the Grade profile SCHADS Award level 5.1
Key Responsibilities:
- Develop, monitor, and review individualized case plans, supporting clients in goal achievement through strength-based strategies, active engagement, and decision-making.
- Maintain contact with clients using flexible engagement to build positive relationships and support independence and goal attainment.
- Provide high-quality service through Strengths-Based and Trauma-Informed care models.
- Apply interpersonal skills, including negotiation, conflict resolution, and solution-focused strategies.
- Facilitate case conferences and work with residents and other supports to develop independent living skills.
- Assist clients in accessing long-term housing and participate in exit strategy goal planning.
- Advocate for clients with external agencies to ensure service access.
- Facilitate client access to community resources, including health, housing, and legal services, and collaborate with allied health services.
- Support clients with specialist services, employment, education, and housing, including applications for the Victorian Housing Register.
- Maintain client files per organizational policy, ensuring accurate documentation of engagements, referrals, and critical incidents.
- Ensure accurate client data entry into SAMiS to support program planning and meet data requirements.
- Actively participate in meetings with a focus on client-centred, continuous improvement, and share best practices across the organization.
- Contribute to service reviews and provide input on procedure development to improve service delivery.
- Stay informed on relevant theory, policies, and legislation, and engage in professional development activities.
- Build effective relationships with partner agencies and key stakeholders to provide comprehensive support.
- Participate in supervision, casework reflection, and team meetings.
- Engage in warm referrals for clients exiting crisis accommodation.
- Offer support to students in placements by sharing knowledge and fostering a positive learning environment.
- Communicate effectively and document information according to policies and procedures.
- Analyse complex situations with the team to find practical solutions.
- Model professional behaviour, displaying initiative, fairness, and accountability.
- Assist with critical incidents and collaborate with the residential team to support clients.
About You:
- Degree qualification and higher educations in Social Work, Psychology, or related field
- Relevant experience in the social service sector including case-management
- Able to demonstrate knowledge of homelessness including context, causes, models, and approaches relating to best practice
- Experience working with internal and external stakeholders to both maintain and create new connections in the sector
- A Nationally Coordinated Criminal History Check
- A current and valid Working with Children’s Check
- A valid Victorian Driver's license.
