About the Role:

As a Full-Time/ Part time Primary Care NP at Medical One, you'll be at the forefront of delivering high-quality, patient-centred care within our medical centres. Working collaboratively with our multidisciplinary team, you'll assess, diagnose, and treat a diverse range of acute and chronic health conditions. When not consulting, you'll seamlessly transition into the role of a Practice Nurse, contributing to the efficient operation of the medical centre. Hours/rosters flexible and negotiable.

Key Responsibilities:

Nurse Practitioner Duties:

  1. Conduct comprehensive health assessments, including physical examinations, medical histories, and diagnostic tests.
  2. Formulate and implement evidence-based treatment plans, including prescribing medications, ordering laboratory tests, and referring patients to specialists as needed.
  3. Provide patient education and counselling on disease prevention, health promotion, and management of chronic conditions.
  4. Collaborate closely with General Practitioners, Registered Nurses, and administrative staff to ensure seamless coordination of care and optimal patient outcomes.
  5. Participate in multidisciplinary team meetings, case conferences, and quality improvement initiatives to enhance patient care delivery and outcomes.
  6. Demonstrate a commitment to professional development by staying current with best practices, guidelines, and advancements in primary care and nurse practitioner practice.
  7. Uphold Medical One's mission and values by fostering a supportive work environment, promoting a culture of caring, commitment, and communication, and delivering exceptional healthcare to every patient.

Practice Nurse Duties:

  1. Assist with patient triage, including taking vital signs, recording medical histories, and preparing patients for examination by healthcare providers.
  2. Perform routine clinical procedures, such as wound care, immunizations, and venepuncture, following established protocols and infection control guidelines.
  3. Provide health education and counselling to patients and their families on preventive care, chronic disease management, and medication administration.
  4. Maintain accurate and up-to-date medical records, including documenting patient encounters, test results, and treatment plans in electronic health records (EHR) systems.
  5. Support the efficient operation of the medical centre by assisting with inventory management and administrative tasks as needed.
  6. Collaborate with healthcare team members to ensure continuity of care and facilitate smooth patient flow within the medical centre.
  7. Demonstrate a commitment to upholding Medical One's mission, values, and standards of excellence in patient care through professional conduct and adherence to established protocols and guidelines.

Qualifications and Requirements:

  1. Current registration as a Nurse Practitioner with the Nursing and Midwifery Board of Australia.
  2. Advanced clinical knowledge and skills in primary care, with a minimum of 1 year of experience as a Nurse Practitioner preferred.
  3. Excellent communication and interpersonal skills, with the ability to effectively collaborate with patients, families, and healthcare team members.
  4. Strong critical thinking, problem-solving, and decision-making abilities, with a commitment to evidence-based practice and continuous quality improvement.
  5. Ability to work autonomously and as part of a collaborative team in a fast-paced clinical environment.
  6. Current CPR certification and professional indemnity insurance coverage.
