As a registered NDIS Support Coordination service, roundsquared provides flexible and responsive Support Coordination to people living with disability and their families. We have two positions available that would be highly suited to an independent Support Coordinator and their Participants seeking a registered established provider to join, however this is not a requirement of the position.
This positions offered are located in Nowra and the Eurobodalla Shire.
Remuneration will be determined based on skills and experience in accordance with the SCHADS Award (Level 3.1 - 4.1 depending on experience). Professional development opportunities and supervision will be provided
We are seeking an experienced person to join our dynamic team supporting NDIS participants to live a good life.
The role:
You will be working with NDIS participants and their families to develop the skills to enable each participant to live a rich, more inclusive and independent life.
You will be assisting participants and their families to identify the services they require, connecting them with relevant services, and ensuring those services are providing effective support.
What you will do:
- You will be responsible for coordinating supports as articulated in the participant’s NDIS plan and will facilitate participants to engage with informal and mainstream services.
- Locate, coordinate and facilitate a range of supports and support providers to meet identified needs.
- Coordinate participants' plans to enable greater independence as well as increased social and community participation.
- Monitor & evaluate services provided to the Participant by external stakeholders.
- Ensure supports are working towards Participant’s goals.
- Work collaboratively with Participants, families, providers & other stakeholders.
- Attend community and networking events.
Skills and experience:
- You have an understanding of what leading a ‘valued life’ for a person with a disability means.
- Qualifications of social services or other relevant fields preferred.
- Ability to work flexible hours to suit the needs of the people we support.
- Experience working with children and/or adults with disability.
- Proficiency in technology, excellent time management and strong administration skills.
- Reliable vehicle and ability to travel in and around Shoalhaven/Eurobodalla Shire regions.
