We are looking for a motivated, dynamic, and detail-oriented Coordinator (care Partner) to join our team at our Spring Hill office. We are seeking someone who has demonstrated experience as a Home Care Package Coordinator and genuinely enjoys providing case management services for people over the age of 65 years.

Thriving in this role requires:

  • Experience in home care package coordination: You'll be experienced at navigating the intricacies of home care, including managing budgets and ensuring efficient and effective service delivery and
  • A passion for client care: Building genuine relationships with your clients and their families will be paramount. Your expertise will empower them to live independently with the support they deserve.

In addition to all of the the above, you will also need:

  • Demonstrated working knowledge of the Aged Care Quality Standards and compliance requirements for Home Care Packages
  • Proven ability to work effectively with colleagues to achieve the best outcomes for our clients
  • Good computer skills including with care databases, word, excel, outlook
  • National Police Check
  • Current Australian Drivers Licence
  • Current First Aid and CPR
  • Right to Work in Australia permanently with no restrictions

Pay: $75,000.00 – $85,000.00 per year
