Role
As a Care Coordinator, you will be responsible for the individualised care, support and services for consumers including the coordination, scheduling and implementation in accordance with approved funding, guidelines, legislative requirements and organisational work instructions.
You will manage care and support which enables the consumer to live safely and independently at home while maintaining wellness, safety and connectedness, in line with the consumers co-designed Care Plan and within the scope of practice of the role.
About You
- Demonstrated experience in Community Aged Care.
- Experience managing Home Care Package and Commonwealth Home Support Programme client is essential.
- Sound understanding and working knowledge of the Aged Care Quality Standards
- Knowledge and experience about the My Aged Care system and pathways
- Ability to communicate sensitively, openly and effectively with consumers, their families/carers, key stakeholders, and other staff
- Proven ability to effectively work autonomously and within a team environment
- Competence in use of technology including smart phones and laptops etc.
- Literacy in use of computer software including Microsoft Office (Word, Excel and Outlook), intranet, online learning systems, consumer databases etc.
Additional requirements
- Minimum qualification of diploma in Human Services / Health / Community / Disability or equivalent, or extensive experience working in a similar role (essential)
- Current National Police Clearance (essential)
- Current First Aid Certificate (desirable)
- Current SA Drivers Licence (essential)
- Reliable vehicle and comprehensive vehicle insurance (essential)
- Current DHS NDIS Worker Screening Check (essential)
- Current Covid-19 Vaccination
