  • Opportunity to be part of an experienced Social Work Team that works across a range of areas including the Community Palliative Care Service, Inpatient unit and Statewide Service for people with progressive neurological conditions.

About the role

Working as part of a comprehensive multidisciplinary team, you will provide Social Work services in the Community Palliative Care Service including psychosocial assessments, supportive counselling in relation to adjustment to diagnosis and end of life, advance care planning, assistance with navigating the service system including referral and advocacy, carer support, family violence risk assessments and interventions.

About you

  • Experience in hospital social work, palliative care, progressive neurology and/or aged care/NDIS.
  • Strong knowledge of systems and supports that help people live well with advanced illness and/or ageing
  • Ability to work autonomously and as part of a team
  • Tertiary qualification in social work
  • Eligible for registration with the Australian Association of Social Work (AASW)
  • A desire to join a team committed to the care of others in a mission based organisation.
