About your opportunity:
In this position you will work under minimal supervision with Cairns High Risk Foot and Diabetes Team within a multidisciplinary health care setting to provide clients with a high standard of Diabetes support and assistance to self-manage aspects of their life that may include planning, access and involvement in community services and decision-making, delivery of specific health care programs, assessing and treating clients, maintaining health care systems and providing health education programs, assisting telehealth services and community liaising functions in order to contribute to better health outcomes for Aboriginal and Torres Strait Islander people.
About your role:
- Act as a cultural liaison to advocate, liaise, network and refer to appropriate Non-Government Organisations, Aboriginal and Torres Strait Islander Liaison Officers and the Multidisciplinary Team in collaboration with and under direction of Registered Nurse.
- Provide effective support and advocacy for identified Aboriginal and Torres Strait Islander High Risk Foot and or Diabetes clients by conducting timely home visits, case management/conferencing, follow-up and referrals to relevant health services/programs.
- Empowering consumers to make informed health care decisions through consultation, education and information sharing.
- Maintain accurate and timely documentation through the provision of monthly activity reports, client file documentation and monitoring Aboriginal and Torres Strait Islander identification records.
- Identify health needs in High Risk Foot and Diabetes services and participate in health promotion programs, appropriate referral, advice and information.
