Your Opportunity

Support vulnerable people at risk of homelessness - not just during a crisis - but throughout their journey towards independence .

Wattle House’s Housing Support Services provides individuals and families who are homeless or at risk of homelessness with wraparound support to secure and sustain long-term, stable accommodation. The service works closely with clients to address barriers to housing, employment, education, and wellbeing while providing culturally safe, trauma-informed, and person-centred care.

As a Housing Case Manager, you will play a vital role in assisting clients to overcome the challenges preventing them from maintaining housing. This will involve working closely with clients, housing providers, and support services to develop and implement case plans that support their long-term stability and independence.

Your Key Responsibilities

  • Respond to and assess referrals from internal and external sources, including NPAH, NAHA, FDV, and AOD services.
  • Conduct intake assessments and risk planning to determine client suitability for housing support program.
  • Develop and review individualised support plans that address client needs and goals, including tenancy, financial stability, employment, education, and social integration.
  • Provide intensive case management, ensuring regular reviews and follow-ups to track client progress.
  • Conduct home visits to support clients in maintaining their tenancies, including assistance with household setup, tenancy responsibilities, and community integration.
  • Arrange and attend collaborative case management meetings with internal and external services, including mental health, AOD, FDV, and family support providers.
  • Maintain comprehensive case notes, reports, and statistical data to ensure accurate documentation and compliance with service requirements.
  • Attend local network meetings to strengthen partnerships and advocate for improved housing support services.

Requirements for Success

With experience in the housing, homelessness, and/or community services sector, you will bring strong case management expertise and an understanding of the challenges faced by individuals and families experiencing homelessness.

  • Certificate IV or Diploma in Community Services, Social Work, or a related field (or equivalent experience with at least three years of case management experience).
  • Strong understanding of housing support services, and homelessness issues, including AOD, FDV, mental health, and financial hardship.
  • Demonstrated ability to advocate for clients with government agencies, housing providers, and community services.
  • Experience working with Aboriginal and Torres Strait Islander communities, including knowledge of cultural systems and protocols.
  • Strong skills in risk assessment, crisis intervention, and trauma-informed care to support individuals with complex needs.
  • Excellent time management and organisational skills, with the ability to manage competing priorities and meet deadlines.
  • High-level written and verbal communication skills, including case notes, reports, referrals, and stakeholder engagement.
  • Proficiency in Microsoft Office and case management systems for documentation and reporting.
  • A full, unrestricted driver’s licence and the ability to conduct home visits and attend meetings across locations.

Before starting work with us, you will need to undertake a national police history check, qualifications, and referee checks, and have a clearance to work with children/vulnerable people.
