About the Role: As a Dementia Care Adviser, you will be at the heart of our mission to provide quality, person-centred support, advice, and advocacy to people living with dementia and their families. You will also play a crucial role in raising awareness about dementia through community promotion and education.
Maximum term, part time position till 21st April 2026, 5 days a week, 5 hours per day based in Mt Ousley Office Wollongong.
What You'll Do:- Empathy and Respect: Treat all clients and their carers with the utmost empathy and dignity, ensuring they have the choice to live their daily lives in a way that is special and unique to them.
- Case Management: Operate case management in line with our policy manual and Standards Guide.
- Support and Advocacy: Provide evidence-based support, advice, and advocacy to people living with dementia and their family carers during one-on-one home visits or in group settings.
- Educational Support: Offer educational and psychosocial support to the families of those living with dementia.
- Community Awareness: Raise awareness in the local community about living with dementia by delivering talks and educational events across the region.
- Customer Service: Foster a culture of exceptional customer service within the team, focusing on what is important to clients and their carers.
- Care Plans: Complete and enter Assessments and Care Plans into HCM within the first 5 days of admission, regularly reviewing and revising them to ensure the service/program offering is optimised, effective, and efficient.
- Budget Management: Efficiently manage budget requirements and finances, ensuring services operate within allocated budgets.
- Administrative Excellence: Handle all reporting and administrative matters promptly and accurately to meet team and organisation requirements.
- Stakeholder Relationships: Develop and maintain positive working relationships with all stakeholders, ensuring effective communication, confidentiality, and diversity awareness.
- Christian Values: Uphold our Christian values and desired culture by working in accordance with our Vision, Mission, and Values, ensuring these are practiced and promoted.
- Diploma in Community Services/Dementia Care or appropriate equivalent qualification is essential.
- Understanding of dementia, its progression, and its impact on the person and their carer, with a demonstrated ability to work with older people, people living with dementia, and their families/advocates.
- Previous experience in Home Care/Community/Aged Care.
- Superior skills in communication, including written and presentation skills.
- Current C class driver’s licence and vehicle.
- Intermediate computer skills in MS Office and client database systems.
