- $1,128.60 - $1,140.30 per week (pro rata) | WHA4
- $1,106.10 - $1,117.60 per week (pro rata) | WHA3

We are looking for a dedicated Community Support Worker to join our team, where you will play an essential role in the Consumer Directed Care model for aged care, disability, and support services. About the Role:
As a Community Support Worker within the Consumer Directed Care model, you will work closely with consumers to provide high-quality, personalized services that support them to maintain independence in their own homes or Residential Aged Care facilities. You will assist with personal care, cleaning, meal preparation, transport, and social support, tailoring services to the unique needs of each individual.
Your role will be integral to ensuring consumers have greater control over the care they receive, including determining service shifts, duties, and provider choices. You will collaborate with consumers, their families, and health professionals to ensure care plans are met, working flexibly and autonomously within the community to deliver the best outcomes for those in need.

What We’re Looking For:
WHA4
- Completed AQF Certificate III relevant to the position.
- Proven experience in exercising own judgment and initiative in the day to day execution of a position.
- Experience in the provision of a direct care service in a health related field.
- Experience in dealing with aged, frail or disabled persons.
- Experience in the use of computer packages e.g. Microsoft Word, Excel.
WHA3
- Current drivers licence and willingness to drive.
- Current Criminal History Check clearances (refer to Criminal History Check check clause).
- Contactable by telephone.
- Access to a reliable, registered and insured (minimum of third party insurance) motor vehicle.
- Meet immunisation requirements as outlined by the Immunisation Guidelines for Health Care Workers in South Australia 2014.

- DHS National Disability Insurance Scheme (NDIS) Worker Check
- National Police Certificate (NPC) for employment involving unsupervised contact with vulnerable groups required for this position (E.g. aged care employment or work involving vulnerable clients)
