About the role
Are you an experienced NDIS Support Coordinator, looking for an exciting short-term contract (3 months), our client is seeking an experienced and dedicated contractor who can is passionate about quality, person-centered support and is committed to upholding the rights of NDIS participants.
As a NDIS Support Coordinator, you will be responsible for coordinating a range of supports and services to empower our clients and help them achieve their goals.
What you'll be doing
- Developing and implementing individualised support plans for clients to help them access the services and supports they need
- Coordinating a range of supports and services, including allied health, accommodation, daily living skills, social and community participation
- Advocating on behalf of clients to ensure they receive the appropriate level of funding and services under the NDIS
- Regularly reviewing and updating client support plans to ensure they continue to meet their needs
- Collaborating with a wide range of service providers, therapists and other stakeholders to deliver integrated support
- Maintaining accurate and up-to-date records and reporting on client progress
What we're looking for
- Proven experience as an NDIS Support Coordinator, with a strong understanding of the NDIS and the supports and services available
- Excellent communication and interpersonal skills, with the ability to build rapport and work collaboratively with clients, their families and a wide range of stakeholders
- Strong organisational and time management skills, with the ability to juggle multiple tasks and priorities
- A genuine passion for empowering people with disabilities and helping them to achieve their goals
- Relevant qualifications in a related field, such as social work, occupational therapy or disability services
