Join UnitingSA's Community Aged Care team as the CHSP (Commonwealth Home Support Programme) Care Partner. The role is responsible for coordinating services that promote and maintain client independence while supporting clients in achieving their goals through client-focused assessment, planning, and review. This role primarily focuses on clients with an appropriate level of complexity, including those funded through the Commonwealth Home Support Programme (CHSP) and Home Care Package/Support at Home clients.
The CHSP Care Partner ensures that all services are aligned with approved funding, legislative requirements, and organizational protocols.
Key Responsibilities:- Work in partnership with consumers, key stakeholders and their carer/advocate to co-design individualised care, support and services for consumers through planning, coordination, scheduling and implementation of personalised and detailed goal-oriented Care Plans.
- Regularly monitor personalised consumer budgets according to program and funding requirements, and organisational processes.
- Assist clients to access services and support through MyAged Care.
- Set realistic expectations for consumers of support they can receive within the scope of their services – underpromise and over deliver.
- Meet with potential client (and consumer representative if applicable) to discuss client's care needs, advise areas in which program can assist, and answer their questions.
- Actively participate in achieving high quality outcomes using a consumer-directed care framework.
- Assist customers to navigate the aged care system to ensure they receive the best possible range of care to meet their needs to remain independent and at home and be an active participant in their community.
- Establish and maintain effective relationships with key stakeholders.
- Escalate high risk situations and incidents within a timely manner and in accordance with UnitingSA Policies, Procedures and Work Instructions.
- Minimum qualification of Certificate IV in Ageing Support or equivalent qualification.
- Experience effectively managing competing priorities to deliver outcomes to a high standard within agreed timeframes.
- Experience supporting carers and advocates of consumers.
- Experience in using electronic rostering systems and consumer databases.
- Sound understanding and working knowledge of Commonwealth Home Support Program (CHSP) as well as the Home Care Packages program.
- Understanding of the Aged Care Quality Standards.
- Knowledge of the My Aged Care system.
- Literacy in use of computer software including Microsoft Office (Word, Excel and Outlook), intranet, online learning systems, consumer databases etc.
- Ability to learn and apply new electronic systems and processes effectively.
- Ability to apply knowledge and skills to a broad range of complex tasks.
