Seeking an experienced Hospital Social Worker for a 10-week Locum contract, based along the south coast of VIC.

On behalf of our client in VIC we are seeking an experienced Level 2 Hospital Social Worker for a 10-week Locum contract. Providing comprehensive psychosocial assessments for all clients referred, involving their families and carers as appropriate in facilitating discharge planning, as well as developing goal-based care plans. Working fulltime as part of a multi-disciplinary team up to 38 hours per week.
Key Responsibilities
- Engage with clients from a variety of social, economic, religious and cultural backgrounds
- Identify cultural, religious, emotional, physical, cognitive, psychological and educational needs, strengths and barriers for clients and their families and carers
- Provide a comprehensive psychosocial assessment for all clients referred, involving their families and carers as appropriate
- Develop goal-based care plans with all clients
- Implement necessary changes to care in collaboration with the client/family/caregiver and multidisciplinary team
- Provide responsible, professional and timely assistance and support to patients, their carers and family members concerning the processes involved with ACAS assessments and transition to Residential Aged Care facilities
- Provide information, support and counselling to palliative clients and their families
- Promote and facilitate client involvement in decision-making, care planning and discharge planning
- Adhere to all professional, legal and ethical guidelines to ensure excellent client and carer outcomes
- Evaluate client and carer responses to intervention and communicate impact to the multidisciplinary team, including the client's GP
- Provide timely and accurate communication of client information with all members of the healthcare team, including the GP
- Assess and identify legal issues involved in competency and guardianship, liaising with relevant governing bodies and implementing referrals where appropriate
- Recognise and report signs and symptoms of abuse, neglect and/or exploitation
- Manage and report risk and actively work towards implementing risk reduction strategies
- Serve as an advocate for clients, families and carers where necessary
- Establish effective relationships with patients/clients, their family / carers, service providers and other stakeholders
About You
- Bachelor of Social Work or Master of Social Work
- Eligibility for membership of Australian Association of Social Workers
- A minimum of 2 years' experience as a qualified Social Worker
- Clinical experience within the public health sector
- Demonstrated clinical skills, knowledge and reasoning in psychosocial assessment and goal-based interventions in an acute, sub-acute and / or community setting
- Excellent interpersonal and communication skills, including proven ability to interact with consumers and their families in a caring and sensitive manner
- Demonstrated commitment to person centred practice
- Demonstrated ability to work both independently and as part of a multidisciplinary team
- Experience in accessing a variety of community resources and systems
- Established skills in organisation, planning and priority setting
- Demonstrated computer skills and accuracy in data entry
- Driver's licence
- A Police Check and a Working with Children check that is current and dated within 12 months of issue
