Sirv is looking for an exceptional Team Leader to join our team in HORSHAM , Victoria, overseeing our Supported Independent Living (SIL) home and ensuring high-quality care for our participants.
About the Role
As a Team Leader, you will:
✅ Oversee the daily operations of our Supported Independent Living homes.
✅ Develop and implement support plans and risk assessments tailored to participants’ individual needs and goals.
✅ Ensure restrictive practices reporting is completed accurately and submitted to relevant authorities (e.g., NDIS, DHHS) within required timeframes.
✅ Work collaboratively with families, teams, and external stakeholders to achieve positive outcomes for participants with highly complex support needs.
✅ Guide and support Support Workers, ensuring they provide care in line with support plans.
✅ Lead by example—providing hands-on support to participants and promoting a strong, person-centered care approach.
About You
We’re looking for someone who has:
✔ Experience in supporting individuals with disabilities, particularly those with complex needs.
✔ A Certificate IV in Aged Care, Disability, or Community Services (or equivalent), along with a current First Aid/CPR certificate.
✔ Proven leadership skills in managing and supervising teams in SIL homes.
✔ Training & experience in Manual Handling.
✔ Experience working with individuals with Intellectual Disability, Epilepsy, Schizophrenia (highly regarded).
✔ experience in managing rosters
✔ A valid NDIS Worker Screening Check.
✔ Happy to work in an environment with cats
