The Role

This role, which is based in Darwin and Alice Springs and reports to the Service Delivery Manager – ICSS, the Care Support Planner assumes the pivotal responsibility of evaluating carers’ needs, guiding them through registration and assessment procedures, and subsequently orchestrating their connection to essential services, all in alignment with service delivery guidelines. Additionally, they evaluate carers’ eligibility for various supports offered through the Carer Gateway, encompassing Emergency Respite, Carer Directed Packages, Carer Counselling, Carer Coaching, and Local Peer facilitated groups.

What you’ll be doing

  • Effectively assess carer needs for supports using the Carer STAR tool
  • Develop comprehensive knowledge of available internal services, Partner services and services available through external organisations
  • Acknowledge and prioritize the significance of dignity and the rights of clients to exercise choice and control.
  • Participate in team meetings and attending training as required
  • Provide information, advice and support to clients and their families to facilitate their personal goals
  • Maintain client records management by inputting relevant data into the CRM system

What we are looking for

  • Have prior experience in intake and assessment services.
  • Knowledge of Services and Service providers throughout the Northern Territory
  • Demonstrate an understanding of consumer directed care
  • Have hands-on experience in collaborating with individuals from diverse cultural backgrounds.
  • Empathy and understanding
  • Sound interpersonal and communication skills both written and verbal
  • Effective computer skills (Word processing, database, Internet, email)
  • Demonstrated ability to apply a person centred or strength-based approach
  • Experience negotiating with community-based organisations or group
  • Willingness to travel

To be considered for the role, you will also need to meet the mandatory compliance requirements which includes:

  • NDIS Worker Screening Clearance
  • Working with Children Clearance (Ochre Card)
  • National Police Clearance (dated within 6 months or less)
  • Current First Aid & CPR certificate
  • NT Drivers License
  • Evidence of Australian Work Rights (current visa or Australian Birth Certificate)
