About the role

Permanent part time opportunity to apply Social Work skills and knowledge in the delivery of psychosocial care to consumers and their families/carers of the Community Palliative Care service. The service identifies and supports the individual and their family with their physical, emotional, spiritual, or social needs.

About you

Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:

  • Actively participate in evidence-based practice initiatives and apply clinical evidence to practice supporting continuous improvement of service delivery.
  • Actively participate within the discipline of Social Work and the multidisciplinary team clinical environment to ensure Social Work clinical services are integrated, efficient and equitable for the community serviced.
  • Actively participate in the identification, development and implementation of quality improvement activities for the delivery of Social Work within the Community Palliative Care service and Social Work discipline.
