You'll provide clinical leadership, support a multidisciplinary team, and coordinate care for patients transitioning from hospital to home. What your day will look like….

As a Clinical Nurse - Case Manager, you'll start your day by assessing the needs of older patients, determining their eligibility for the Transition Care Program. You'll engage with a multidisciplinary team, using your advanced clinical judgment and problem-solving skills to manage complex cases. Your day will involve liaising with clients, their families, and various health professionals to ensure seamless care delivery. You'll also participate in quality improvement initiatives, mentor junior staff, and contribute to a positive workplace culture. Every interaction you have will be focused on achieving the best possible outcomes for your patients, making a real difference in their lives.
