About the Role:
The Integrated Primary Care program focuses on ensuring access to Allied Health and Specialist Nursing services in the primary health care space across the Yorke Peninsula, Mid and Lower North and Southern Fleurieu/KI LGA. The Care Coordinator works as part of a multidisciplinary team to help ensure the delivery of the IPC program and works closely with all members of the IPC team to support all aspects of program, through referral processing, data collection and collaboration with stakeholders.
Experience Requirements:
- High level interpersonal and communication skills (verbal and written) and consultative skills reflected in an ability to engage and collaborate with, a wide range of service providers and community members

- Ability to capture and share information using client record management systems
Qualification and Other Requirements:
- Relevant tertiary qualifications (primary health care, nursing, health promotion, public health, community development) or combination of relevant training and experience deemed to be appropriate.
- Current or willing to obtain a Working with Children's Check (WWCC)
