Whilst this selection process will initially be used to fill the above vacancy, it may also be used to fill other "similar" vacancies throughout the health service. In addition to this, should the successful applicant decline or vacate the advertised vacancy, then the next most suitable applicant may also be selected from this process. Both of these options remain valid for a period of twelve (12) months from when the authorised delegate endorses the recruitment decision.
Position Profile: The Aged Care Nurse Practitioner (NP) will be based within the Community and Virtual Care Directorate (CVC) and will perform functions of clinical care delivery to older patients with a range of acute and/or complex needs. This Aged Care NP role will deliver care in virtually enable environments, assessing and managing geriatric patients via innovative virtually enabled models of care. The NP will provide expert knowledge and high-level clinical service, including investigations, prescribing of medications and direct referral of patients to other health care professionals. The NP will practice within the approved scope of practice, competence, legislative and policy frameworks, and the strategic directions of the Community & Virtual Care Directorate.
The Aged Care NP will work collaboratively with medical staff, nursing staff and other health professionals within the inpatient, outpatient, or community settings. The full scope of this innovative NP role will evolve with collaborative interface from Geriatricians. This newly created Aged Care NP role will be at the forefront of shaping virtually enabled models of care for older adults residing in the EMHS catchment.
