About the Role
As a Case Worker you will assist our young people to address previous homelessness issues and support them in crisis accommodation to transition to longer term housing options. The role requires the worker to work alongside our Youth Workers in a 24/7 service.
Specifics of the role

Work with clients to develop individual case plans that are trauma informed, and person centered.

Monitor and review client’s progress

Provide clients with a range of support options to enable them to make informed decisions on their safety and well-being.

Identifying relevant services and resources (formal and informal) to make warm referrals and ensure client linkage to long term support services

Accurately complete case notes, risk assessments and other necessary administrative tasks
What you will need
-
Minimum Cert IV Youth Work or similar
-
Experience as a Youth Case Worker

Strong communication and organisational skills
-
Ability to prioritise work and respond to deadlines
-
Current WA ‘C’ Drivers Licence
-
Current Working with Children Card
-
Current First Aid HLTAID003, or willingness to obtain.

A Nationally Coordinated Criminal History Check obtained in last 3 months

Proven capacity to work independently as well as with a team
