We are seeking a dedicated and skilled Senior Social Worker to join our Older Persons Mental Health Services (OPMHS) Team.
The Senior Social Worker undertakes clinical care coordination as a part of the multi-disciplinary team within the Older Persons Mental Health Service. The role is responsible for the delivery of a range of clinical activities, including mental health and psychosocial assessments, risk assessments, treatment and care planning, therapeutic interventions, advocacy and liaison with other community service providers.
About the role:
As the Senior Social Worker, duties for the role will include:
- Undertaking assessments and planning recovery focussed services for individual consumers, including bio-psychosocial assessment, mental state examination and risk assessment of consumers
- Implementing and evaluating the impact of a range of therapeutic interventions
- Working closely and developing effective partnerships with government and non-government agencies that may be involved in consumer care
- Participating and facilitating staff development sessions and programs and when appropriate provide education, induction and orientation
- Supporting the Team Manager to develop and implement work flow processes, quality assurance processes and ensure optimal work outcomes
About you:
- Ability to work effectively within a multidisciplinary team
- Advanced skills in undertaking a comprehensive bio-psychosocial assessment including risk assessments of consumers presenting with high or complex needs
- Knowledge and experience of evidence based theoretical frameworks relevant to the field of mental health (such as Cognitive Behavioural Therapy, Motivational Interviewing, Family Therapy, Safety Planning/Relapse Prevention, and the Recovery Model)
- An appropriate degree or equivalent qualification which gives eligibility for full membership of the Australian Association of Social Workers
- Demonstrated competence in all the competency standards for social workers working in mental health
- Proven experience in delivering high quality and safe care consistent with the National Safety and Quality Health Care Service Standards
