Who We're Looking For
We welcome candidates with minimal or no experience in support coordination. All we require is relevant industry experience for example support work or qualifications, we also require the candidate to have strong administration skills.
Role Overview
You’ll begin by managing a small caseload of approximately 10 clients, expanding your responsibilities as you grow in confidence and expertise. The role includes 5-10 hours of administrative work per week, ensuring a smooth transition into your support coordination duties.
Key Responsibilities
Administrative Tasks:

Draft and complete service agreements and consent forms.

Attend and document meetings through detailed minutes.

Research local allied health and support providers for client needs.

Send welcome emails to new clients, fostering strong relationships from the start.

Manage the admin email for inquiries and correspondence.

Be available during business hours for phone diversion as needed.

Update client support coordination plans for efficient service delivery.

Maintain accurate client records by managing database inputs.
Support Coordination Tasks:

Assist NDIS participants and families in achieving their goals and optimizing their plans.

Build strong relationships with service providers and allied health professionals.

Focus on capacity-building and enabling choice and control for participants.

Prepare comprehensive plan review reports for the NDIA.

Develop and maintain sound plan implementation practices.

Empower participants to understand and implement their NDIS plans.

Collaborate with internal and external teams to advocate for participant needs.

Innovate and refine processes in line with best practices and NDIS guidelines.
Details
Job Type: Subcontract
Pay: $45.00 – $60.00 per hour for the Support Coordination role.
Expected Hours: 5–15 hours per week, with room to increase ours as your caseload builds.

Requirements

Preferred Certifications: Working with Children Check, Driver’s License, cert III or above in disability, aged or community services, or other relevant qualifications.
