Description:
As a NDIS Support Coordinator, you will play a crucial role in helping participants navigate their plans, connect with services, and achieve their goals.
Days/Hours: 6 hours /day 5 days per week – flexible work arrangements.
Key Responsibilities
- Planning & Goal Setting: Collaborate with participants to develop personalized plans that align with their goals and support needs.
- Service Coordination: Connect participants with the appropriate service providers, ensuring access to necessary supports.
- Monitoring & Review: Conduct regular check-ins to assess the effectiveness and financial sustainability of services and adjust plans as needed.
- Problem Solving: Address and resolve issues with service providers, ensuring that participants receive the supports they need.
- Advocacy: Empower participants to make informed decisions about their care while advocating for their rights and needs.
- Plan Implementation & Documentation: Assist in implementing NDIS plans and maintain accurate records of services, meetings, and progress.
- Stakeholder Liaison: Work with participants, families, and external service providers to ensure coordinated, holistic person-centred support.
- Collaborate: Assist office staff with administrative tasks such as rostering, compliance and training.
Key Requirements
Skills, Attributes & Experience
- Strong knowledge of the NDIS framework, legislation and experience navigating the NDIS environment.
- Experience working in disability services with participants, their families and carers to identify and access opportunities which support the achievement of goals.
- Ability to network with other relevant support agencies such as correctional or justice services, accommodation services, drug and alcohol support agencies, mental health support services.
- Proven ability to plan, prioritise, meet deadlines and juggle competing demands whilst working in a fast pace and flexible environment.
- Well-developed communication skills (both oral and written) with the capacity to prepare reports and present information in a manner appropriate to the purpose and audience.
- Well-developed interpersonal skills with the ability to work independently and collaborate effectively with a wide range of colleagues, participants and service providers.
- Proficiency in Microsoft Office, Teams and participant management software and rostering systems.
- Knowledge of Workplace Health & Safety (WH&S) policies and procedures.
- A person-centred approach with a commitment to supporting independence and improving the quality of life for participants.
Licences / Certifications & Qualifications
- Relevant qualifications (Social Work, Disability, Community Services, Mental Health etc.) preferred or extensive experience working in disability services, social work or a similar environment.

- NDIS workers screening check or willing to obtain (mandatory)
- First aid certificate or willing to obtain (mandatory)
- Working with Children Check or willing to obtain (mandatory)
