About the role: Lifestyle Facilitator (Support Worker) Lifestyle Facilitators (Support Workers) are responsible for implementing support services aimed at maximising the living, employment, social and recreational skills of people with a disability. Community inclusion, independence, decision making, and personal choice should be promoted through these supports, with a focus on individual needs.

  • Strong communication, interpersonal and advocacy skills including the ability to work and communicate effectively with managers, office staff and lifestyle facilitators, Participants and other stakeholders.

Experience:

  • Demonstrated understanding of disability, mental illness, and the needs of carers of and Participants.

  • Experience engaging with people who may have complex needs and behaviours.

  • Strong coaching and mentoring skills

Required Certifications:

  • Queensland Provisional Green or above Driver’s License and comprehensively insured vehicle.

  • Working with Children Check (Blue Card)

  • Disability Worker Screening/NDIS Worker Screening (Yellow Card)

  • Completion of NDIS Worker Orientation Module

  • Nationally recognised First AID & CPR Certificate

  • Two doses of the Covid vaccination

Desirable Knowledge and Skills:

  • Experience working in Disability or Community Services sector.

  • Experience working within supported independent living (SIL) residential environments.

  • Ability to develop and maintain networks to a diverse range of service providers and client groups.

  • Understanding of privacy and confidentiality issues.

    Duties:

  • Assist Participants to engage within the community, support personal development and endorse positive behaviour plans

  • Take part in household tasks, meal prep, tidying, basic cleaning.

  • Support participants who exhibit challenging behaviours and mental health conditions via understanding and implementing Positive Behaviour Support Plans

  • Supporting participants with epilepsy and complex needs

  • Undertaking personal cares; included but not limited to showering, toileting and dressing

  • Manual handling: included but not limited to hoist transfers, slide sheets and wheelchairs

  • Support Participants to reach their goals as outlined within their NDIS plan

  • Adhere to relevant industry legislation and standards.

  • Work within abbaCare’s Policies and Procedures to ensure best practice
