About the role:
We are seeking a dedicated and compassionate Pharmacist to join our dynamic healthcare team in Central Coast, New South Wales.
Reporting to the Team Lead, you will be working collaboratively to deliver a holistic and integrated suite of services to Australia’s healthcare sector.
The Credentialed Pharmacist role with Choice Aged Care is a hybrid role supporting the community with their healthcare needs. This role is essential in ensuring the safe and effective management of medications within residential aged care facilities as well as supporting aging adults in their own homes. You will be responsible for bringing expertise in medication management and pharmacotherapy to provide collaborative and integrated care for all clients as part of their multidisciplinary healthcare team.
This is a flexible role, with permanent full-time, or flexible part time opportunities. The role is located in Central Coast, New South Wales. Travel within your allocated territory will be required as part of the role, in addition regional travel may be available.
Key responsibilities:

Conduct medication management reviews and clinical interventions.

Conduct medication management reviews (MMRs) and home medication management reviews (HMRs).

Be the first point of contact to address medication management enquiries from various stakeholders involved in patient care.

Collaborate and communicate effectively with residents, families, and carers.

Regularly liaise with medical practitioners to conduct medication reviews and reconciliations.

Develop medication management plans in collaboration with the prescriber.

Conduct an analysis of the benchmarking data to form quality improvement initiatives to promote best practice.

Participate and contribute in GP rounds or case conferences.

Support the care of remote and regional communities with RMMR’s and HMR’s.

Provide review for patients receiving end-of-life care and provide recommendations.

Conduct and/or coordinate vaccinations as per state legislation.

Review and identify ways to enhance facility policies and procedures.

Conduct regular audits to ensure compliance with regulatory standards.

Facilitate and/or deliver group or individual education sessions about medicines and medication use for key healthcare professionals, carers and families.

Assist with National Safety and Quality Health Service (NSQHS) compliance and implementation and use of National Residential Medication Chart (ENRMC).

Adhere to the Aged Care Facilities Clinical Governance Framework.
About you:

Passionate about providing high quality services.
-
Current AHPRA registration.
-
Current credentialling pharmacist qualification or willing to complete credentialed pharmacist qualifications during onboarding. Estimated to be completed in 60-90hrs.

Current Training and Assessment (TAE) qualification or willing to complete TAE qualifications during first two years of employment.

Strong clinical knowledge and understanding of pharmacotherapy in elderly patients.

Service and patient care oriented with a proactive approach and consistent follow-through on commitments.

Excellent communication and interpersonal skills.

Ability to work independently and collaboratively within a multidisciplinary team.

Good attention to detail and proficiency in documentation.

Flexibility to travel to various locations as required.
The successful candidate will be required to return a positive Police Clearance.
