J oin our Home Rehabilitation team as a Clinical Nurse and help support our commitment to quality patient care
The Clinical Nurse in the Home Rehabilitation service will provide direct clinical care to consumers of the service who have recently been discharged from hospital. This care may be delivered directly to patients within their home environment or via the use of video conferencing. The Clinical Nurse is required to have advanced clinical assessment skills, excellent communication and problem-solving skills and be able to work within an interdisciplinary team in order to support patient care.
As the Clinical Nurse this role will be predominantly clinical in nature and will be assigned an appropriate portfolio for the Home Rehabilitation Service.
About the role:
Reporting to the Manager of the Home Rehabilitation, Telerehabilitation and Outreach, duties for the role will include:
- Provide proficient, person centred, clinical nursing care and/or individual case management to patients in a defined clinical area
- Monitoring patient care plans to ensure appropriate care outcomes are achieved on a daily basis
- Participate in clinical teaching, overseeing learning experience, and goal setting for students, new staff and staff with less experience
- Plan and coordinate services including those from other disciplines
- Act as a resource person within an area based on knowledge, experience and skills
About you:
- Ability to prioritise workload and meet set timelines, whilst working under minimal supervision
- Ability to work effectively within a multidisciplinary team
- Current Driver’s License
- Demonstrated competence in rehabilitation nursing practice in accordance with the relevant standards
- Registered or eligible for registration as a Nurse by the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate
- Proven experience in delivering high quality and safe care consistent with the National Safety and Quality Health Care Service Standards
To be considered for the RN2C (Clinical Nurse) level you must have at least three years, full time equivalent, post registration experience with experience in the leadership and direction of student nurses, enrolled nurses, and less experienced registered nurses.
