Total potential earnings: $97,500 - $105,500 (Salary + Allowances + Remote Travel Allowance) plus additional benefits detailed below:
- Competitive Salary: Annual salary ranging from $90,000 to $98,000, including various allowances and additional work/on-call allowances.
- Accommodation: Fully furnished staff accommodation is provided, with a small subsidised utilities fee — only $30 per bedroom per fortnight.
- Work Vehicle: Allocated work vehicle with approved personal use when not on leave, supporting both operational needs and convenience.
- Remote Travel Allowance: $7,500 per year, paid in fortnightly instalments.
- Work Phone: Provision of a work phone to support effective communication and role responsibilities.
- Program Leave: Unique 6-week continuous service cycle with 1 week of paid leave, in addition to other leave entitlements.
- Loaded Annual Leave: 4 weeks of annual leave each year, with leave loading.
- Paid Christmas Leave: A separate paid leave for Christmas closure (subject to the business closure period).
- Relocation Assistance: One-time $1,500 relocation support — 50% upfront, balance reimbursed upon receipt submission.
- Fixed-term Contract: Contract until 30th June 2026, potential for extension.
- Save on Tax with Salary Sacrificing: Salary sacrifice up to approximately $16,000 per year.
- Superannuation: Standard superannuation contributions.
- Non-Smoker Incentive: Additional 1% employer superannuation contribution for non-smokers (conditions apply).
Position Overview
Tullawon Health Services is seeking a dedicated Community Connector Lead to oversee a team supporting Elders and community members living with disabilities. This role is responsible for ensuring culturally safe, community-led care through effective referral pathways and coordinated support services, including Elder Care Connector and NDIA Community Connector programs. The successful candidate will advocate for clients, assist them in navigating their entitlements, and ensure access to essential services. They will also maintain quality standards, support accreditation processes, and manage mandatory staff training to ensure a safe and professional environment. This is a unique opportunity to make a meaningful impact in a remote Aboriginal community by enhancing care pathways and improving service accessibility.
Key Responsibilities
- Lead the Community Connector Team, facilitating regular team meetings and ensuring collaboration across Aged Care, NDIA, and Disability services.
- Ensure culturally safe, needs-based support for Elders and disability clients, helping them navigate service entitlements and access appropriate care.
- Oversee best-practice referral processes, both internally and externally, to ensure seamless access to aged care services
- Oversee the connecting of participants and development of individualised plans suitable for the NDIS.
- Ensure all services meet accreditation, compliance, and quality review standards, playing a key role in audits and reporting.
- Oversee mandatory training coordination, identify staff development needs, and facilitate learning opportunities for the team.
- Maintain accurate consumer records, oversee data collection, and prepare quarterly reports to track KPIs and service outcomes.
- Participate in SAWCAN network meetings, share resources, and collaborate with internal and external partners to enhance service delivery.
- Support clients in assessments, care planning, and provider selection while advocating for their cultural and care needs with service providers.
- Promote and enforce WHS standards, address workplace hazards, and support staff mental health and wellbeing.
- Document and report incidents, risks, and complaints, ensuring appropriate follow-up and resolution in line with policies and procedures.
Eligibility
- This job opportunity is not a remote or work-from-home position; it requires the selected candidate to work on-site at the company's location.
- Must be willing to relocate and live in the South Australian Aboriginal Community called Yalata.
- Must be over 18 years of age.
- Must have a current Australian driver’s licence and the ability to drive a manual vehicle.
- Must be able to pass a police check assessment.
- Must hold a valid Working with Children's Clearance.
Qualifications and experience
- Diploma in Community Services or a related social field.
- Experience in connecting participants and developing individualised plans suitable for the NDIS.
- Experience working with Aboriginal and Torres Strait Islander people, particularly in aged care or disability services.
- Strong communication and interpersonal skills, with the ability to build trust within the community.
- Ability to work autonomously, manage priorities, and meet deadlines within established policies and procedures.
- Commitment to culturally safe, person-centred care and supporting vulnerable people.
- Understanding of the barriers faced by remote Aboriginal communities in accessing health and aged care services.
